Terms and Purchase Policy

1. Your agreement

These terms, the Participant Waiver and Medical Acknowledgment, the Privacy Policy, and the price and benefits shown at checkout form your agreement with Baseline Recovery Studio. Do not complete checkout until every field is complete and you have read the full agreement. Your checkout consent is your electronic signature.

NOTICE TO THE BUYER: DO NOT SIGN THIS CONTRACT UNTIL YOU HAVE READ ALL OF IT. ALSO, DO NOT SIGN THIS CONTRACT IF IT CONTAINS ANY BLANK SPACES.

2. Membership charges and renewal

Monthly memberships renew automatically through Square at the price and billing frequency shown at checkout until you cancel. You authorize Square to charge your selected payment method for each renewal and any applicable taxes. Square processes payment card details. Baseline does not store your full card number.

Membership benefits, included session credits, and access rules are the benefits shown for the selected plan. Unused monthly benefits do not roll over unless the offer expressly says they do. A failed payment may pause booking access until billing is current.

3. Membership cancellation

You may cancel your membership at any time by providing 30 days’ written notice. Any scheduled payment that falls within those 30 days will still be charged, and you may continue using your membership through the end of the paid period.

Send written notice to [email protected] or 111 Dees Dr., Madison, MS 39110. We do not issue a partial-month refund after a billing period begins, except when required by law or when Baseline made a billing error.

If death or disability prevents the member from using the services, future payment obligations end and Baseline will refund the unused prepaid portion as required by Mississippi law.

4. One-time purchases, bookings, and refunds

The checkout page identifies whether a purchase is recurring or one time. Completed sessions and redeemed access are nonrefundable, except when required by law. If a member cancels a future session before it begins, the unused session credit returns to the membership. A completed session or no-show does not return a session credit.

If Baseline cancels a paid session and cannot provide a reasonable replacement, Baseline will restore the session credit or refund that session. A gift redemption link may expire for security, but expiration of the link does not erase a paid, unused gift. Contact the studio for a replacement link.

5. $20 first-visit membership credit

A first-time guest who purchases the $20 First Recovery Session and buys an eligible monthly membership within 30 calendar days receives one $20 credit toward the first membership payment. Use the same email address for both purchases so the system can match them. The credit has no cash value, cannot exceed the first membership payment, and may be used once.

6. Participation and studio rules

Follow staff instructions, safety screening, posted equipment rules, appointment times, and the Participant Waiver and Medical Acknowledgment. Baseline may stop a session when continued participation appears unsafe or when a participant does not follow studio rules.

7. Changes and contact

Baseline may update these terms for future purchases. A material change to an active recurring membership will be communicated before it applies. Questions and written notices may be sent to [email protected] or 111 Dees Dr., Madison, MS 39110.
