Waiver and Medical Acknowledgment

Recovery services involve heat, cold, pressure, light, water, and physical exertion. This acknowledgment explains your responsibilities and the inherent risks you accept when participating.

General wellness services

Baseline provides general wellness and athletic recovery services. Baseline does not provide medical care, diagnosis, treatment, or medical advice. Website and staff information is educational and does not replace advice from a licensed health professional. No result is promised or guaranteed.

Your health and eligibility

You confirm that you are able to participate safely. Ask a licensed health professional before participating if you are pregnant, have a cardiovascular, blood pressure, respiratory, neurological, seizure, circulation, skin, eye, or heat-regulation condition, have an implanted electronic or metal device, recently had surgery or an injury, take medication that affects temperature or light sensitivity, or have any other condition that may affect participation.

Tell Baseline staff about relevant conditions, injuries, medications, allergies, or symptoms before a session. Do not participate while impaired by alcohol or drugs. Stop immediately and tell staff if you experience pain, chest discomfort, breathing trouble, dizziness, faintness, numbness, unusual pressure, panic, or any unexpected symptom.

Inherent risks

Depending on the service, inherent risks include cold stress, overheating, dehydration, dizziness, fainting, skin irritation, light sensitivity, pressure discomfort, ear or sinus discomfort, claustrophobia, muscle soreness, slips, falls, and aggravation of an existing condition. Hyperbaric, PEMF, red light, sauna, cold plunge, float, and compression services each have equipment-specific screening and instructions that you agree to follow.

Voluntary participation and assumption of risk

You choose to participate voluntarily. You understand the nature of the selected service and accept the ordinary, inherent risks of participation. You agree to use the equipment only as instructed and to ask questions before starting if anything is unclear.

Release and responsibility

To the fullest extent permitted by law, you release Baseline Recovery Studio and its owners, employees, and contractors from claims arising from the inherent risks of a service or your failure to follow safety instructions. This agreement does not release gross negligence, intentional misconduct, or any right that law does not allow you to waive.

You are responsible for loss or damage caused by intentional misuse of equipment or disregard of staff instructions. If urgent assistance appears necessary, you authorize staff to contact emergency services. You remain responsible for costs charged by outside medical providers.

Minors and electronic signature

A parent or legal guardian must consent for a participant under 18 and accepts these terms on the minor's behalf to the extent allowed by law. Checking the consent box at checkout is an electronic signature and confirms that the participant information is accurate.
